Special Feature A Case of Late Kidney Allograft Failure: A Clinical Pathological Conference from American Society of Nephrology Kidney Week

چکیده
Introduction With the advent of more potent immunosuppression, early outcomes after kidney transplantation have been excellent. However, long-term graft survival has improved at a slower pace and does not reflect the substantial gains made in the early post-transplant period (1). Identification of the failing kidney allograft is based on the detection of alteration in GFR, using serum creatinine as the primary surveillance tool. About half of late graft failure is associated with specific pathology within the kidney allograft and allograft biopsy is a key tool to manage and diagnose late injury (2). In the following clinical case presentation from Kidney Week 2011, we present a case of late allograft failure after an initial early acute rejection and a period of apparent clinical stability. The case is instructive in identifying the causes of late kidney injury and demonstrates the role of allograft biopsy in assisting in the critical management of recipients with declining kidney function. Proper intervention even in the late post-transplant period may alter the course and ameliorate the decline in allograft function.

Congenital Hepatic Fibrosis: An Uncommon Cause of Chronic Renal Failure
Congenital Hepatic Fibrosis (CHF) is a rare disease that affects both the liver and kidneys. Congenital hepatic fibrosis (CHF) is an autosomal recessive inherited malformation defined pathologically by a variable degree of periportal fibrosis and irregularly shaped proliferating bile ducts. Affected individuals also have impaired renal function, usually caused, in children and teenagers, by an...
متن کاملIntroduction to the well-transplant visit--more than vital signs and a creatinine check.
Against the backdrop of remarkable advances in renal transplantation in the past half-century resulting in excellent early patient and allograft survival rates, it is clear that commensurate improvement in long-term outcomes lags. Current research is increasingly focused on improving the longevity and quality of life after transplantation. As prognostic risk factors are identified, new strategi...

The Nephrology Quiz and Questionnaire remains an extremely popular session for attendees of the Annual Kidney Week Meeting of the American Society of Nephrology. During the 2015 meeting, the conference hall was once again overflowing with eager quiz participants. متن کاملAcute renal failure in the transplanted kidney.
Acute renal failure (ARF) in the transplanted kidney represents a high-stakes area of nephrology and of transplantation practice. A correct diagnosis can lead to rapid return of renal function; an incorrect diagnosis can lead to loss of the graft and severe sequelae for the patient. The diagnostic possibilities are many (Fig. 10-1) and treatments quite different, although the clinical presentat...

The case of a 16-yr-old woman who received an ABO-incompatible renal allograft for end-stage renal disease due to membranoproliferative glomerulonephritis type I is presented. The patient's posttransplant course was complicated by cytomegalovirus (CMV) infection and the rare finding of glomerular CMV inclusions on renal biopsy. This article focuses on the pathology and pathogenesis of glomerula...
